All patient data collected by Health Care Providers and entered into the portal (“The Program”) is visible to the prescribing physician and support staff only. We take every possible precaution to make sure information is encrypted and password protected. The portal found at meshmd.com and all associated elements are owned and operated by meshMD Inc. (“Service Provider”)
WHAT PERSONAL INFORMATION IS COLLECTED
a. Information Collection and Use
Service Provider will not sell, share, transfer or rent any personal information to others in ways different from what is disclosed in this statement. Service Provider collects information from you on the Profile and Registration pages ("Profile pages") on our site. The Service Provider also collects information that you provide about your patients. Service Provider will never sell, disseminate, or disclose your name, date of birth, or medical license number or any personal information about your patients. Service Provider does however reserve the right to collect, disseminate, sell and otherwise disclose non-personal information provided by you.
b. Third Party Email Addresses
For the purposes of inviting and introducing third parties to meshmd.com, you will be given the option to provide email addresses of third parties to the Service Provider. In so doing, Service Provider will not store or disseminate your username or affiliated password nor solicit or spam any third party email addresses so provided. This platform is compliant with CASL - the Canadian Anti Spam Legislation.
c. Unauthorized Users
You must be a licensed physician, nurse, medical student, or health care provider to be a member/user of meshmd.com.
HOW YOUR INFORMATION IS USED
a. Use of Personal Health Information
meshmd.com does collect personal health information about individuals. Patient Personal Health Information is only used so the Service Provider can provide services including, but not limited to, the electronic prescribing of medications and the enrollment of patients into specialty assistance programs.
The Health Insurance Portability and Accountability Act of 1996 (HIPAA) privacy rule, which took effect on April 14, 2003, and similar legislation in other countries, allow for the use of de-identified health information so long as identifiers have been "stripped" and a key is not disclosed that would allow the information to be re-identified. Service Provider does not possess a key to re-identify patient data.
b. Sale of Assets, Merger, Acquisition, Bankruptcy
Information collected from you may be transferred to a third party as a result of a sale or acquisition, merger or bankruptcy involving the Service Provider. Any such third party will be contractually bound by the Privacy Policy.

When you enter meshmd.com, we collect your browser type, domain, and IP address (‘Internet Protocol' address - a set of unique numbers that identifies your computer on the internet). This information reveals nothing personal about you other than the IP address from which you have access to the Site. This information is gathered for all meshmd.com visitors. In addition, we store certain information from your browser using "cookies." A cookie is a piece of data stored on your computer tied to information about the user. We use session ID cookies to confirm that users are logged in. These cookies terminate once the user closes the browser. When Service Provider requests that you enter sensitive information, that information is encrypted using Secure Sockets Layer (SSL) software, AES encryption, or other security protocol, which encrypts the information you input. No administrator at Service Provider will have knowledge of Your password. It is important for you to protect against unauthorized access to your password and to your computer. Be sure to log off from Service Provider when finished using a shared computer. Service Provider also protects your personal information off-line. Access to your personal information is limited to employees who Service Provider reasonably believes will need that information to provide services to you. However, Service Provider is not responsible for the confidentiality, security or distribution of your own personal information by our partners and third parties outside the scope of our agreement with such partners and third parties.
